Part 1. Survey questionnaire

QUESTION 1 Does your company have the necessary business licenses and permits to supply chemicals, consumables, and spare parts?

If yes, please provide supporting documents

QUESTION 2 Does your company have more than three (3) years of experience in supplying chemicals, consumables, and spare parts for laboratory equipment in the Oil and Gas industry or Petrochemical plants?

1.   Yes

2.     No  

If yes, please provide a list and copies of contracts for supplying chemicals, consumables, and spare parts, with a total contract value of at least 9 billion VND in last three years.

QUESTION 3 Shall your company provide CO (Certificate of origin), CQ (Certificate of quality), MSDS, COA for all consumables, spare parts and chemical?

If yes, , please provide a confirmation letter mention clearly to provide CO/CQ/MSDS/CO. In which be specific for CO if issued by manufacturer or Chamber of commercial, original or copy.
QUESTION 4: Do you provide warranty/shelf-life at least 12 months from delivery date to NSRP? Does your company have procedure to handle defect materials or damage claim from customer?
