SURVEY QUESTIONAIRE AND SCREENING CRITERIA Services for Repairing Internal Components of Cooling Tower

Nghi Son Refinery & Petrochemical LLC (NSRP) is looking for service providers who could Design, Fabricate and Remove damaged and reinstall new support, components made by Fiber Reinforced Plastic/Polyvinyl Chloride (FRP/PVC) inside cooling tower including pipes support, pipe branch, nozzles, and other accessories.

Part 1. Survey questionnaires

QUESTION 1: Does your company have a business license and company official representation for fabrication and support oil and gas production/maintenance services following the law?

1.         Yes

2.         No

If yes, please provide the notarized copy

QUESTION 2: Did your company have experience in Design, Fabrication, Remove damage, Installation New support, branch pipe, nozzle …etc. for Cooling tower?

1.     Yes

2.     No

If yes, please provide detailed evidence

QUESTION 3: Does your company supply FRP (Fiber reinforced plastic) Material and PVC (Polyvinyl Chloride) material?

1.     Yes

2.     No

If yes, please provide detailed evidence

QUESTION 4: Do you provide a warranty for such services as stated in Question No.2?

1.    Yes

2.    No

Potential candidate is “Passed “and selected to form the long list if their answers are:

Answered “Yes” – for all questions and No 1; 2; 3; 4 with sufficient evidence.
