• List out at least 02 similar projects completed in recent years (within 5 years for Wear Pads and 10 years for Half Round Bar) in the Oil & Gas / Petrochemical / Power industries by providing at least the following information.

2. Do you have validated Quality Management Certification ISO 9001?

3. Do you have qualified personnel for the performance of the service which includes but is not limited to the following positions?

• Structure engineer
• Inspector
• Supervisor
• Applicator
• Welding supervisor
• Welding inspector
• Safety supervisor

Please kindly provide certifications held by the above positions.

4. Does your company provide equipment to perform the service which includes but is not limited to the following equipment?

• Pipe rack jack/Pipe lifting jack and support tools
• Generator, Compressor…
• Cutting/Welding and grinding tools
• Lifting tools (chain blocks, hoists, jacks, etc.)
• Inspection tools (Humidity testing, Surface comparator, Ultrasonic thickness gauge, calipers, gap gauge)

5. Are your wear pads/half round bars manufactured in-house or procured from other supplier?

6. Does your company have a method statement for wear pads/half round bars installation? Please provide.

7. HSE and Project Experience

• Do you have an HSE plan for wear pad/half round bars installation works? Please attach.
• Do you have PPE management, working at height and Hot work procedure? Please attach.

8. Do you provide a warranty for the installed wear pads/half round bars? If yes, please kindly specify the duration and conditions.

9. Does your company provide the services on your own or with other partners as consortium? Please, kindly provide a detail profile of your partners.
