FOR SERVICE TO SUPPLY AND INSTALL WIRELESS COMMUNICATION

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is preparing for the procurement of the Service to supply and install wireless communication with detail information as Attachment 1- SOW

QUESTIONNAIRE TO FORM SUPPLIER LIST 

1. Do you have ability to supply and install wireless communication with specification (as stated in SOW) to NSRP?

2. Does your company have the necessary business licenses, permits and certificates to provide this service?
   a.Yes
   b.No

If yes, please provide evidences.

3. Does your company have more than 05 years’ experiences (05 years is counted from the first Contract) in providing installation, configuration, commissioning service related to PLC equipment and industrial communication network?
   a.Yes
   b.No

If yes, please provide list of Contract (at least 02 full copies of executed contracts), specifically indicating your experience in PLC and industrial communication network

4. Has your company’s manpower been trained and qualified to carry out the PLC installation, configuration and commissioning work?
   a.Yes
   b.No

If yes, please provide copy of CV of manpower (at least 03 manpower who must have at least 05 years’ experience in PLC/Instrument installation, configuration and commissioning) and training certificate (qualified by manufacturer)
