SURVEY QUESTIONAIRE FOR GENERAL CHEMICAL SUPPLY

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procurement of General Chemical to select suitable supplier for supply the good.

If your company is interested in those procurement packages, please fill in and the complete questionnaire below then send email to hieu.nd@nsrp.com.vn “Response for Questionnaire “Supply of General Chemical” by 7^th April 2025 at the latest. Kindly send responses to survey questionnaire and provide supporting documentation for the response by attaching directly to the email.

QUESTION 2: Does your company have dangerous goods transportation license issued by the Authority?

If yes, please provide copy of the evidence.

QUESTION 3: Please provide the information of your products as Table 1:

1. Product information
2. Technical Data Sheet (TDS)
3. Safety Data Sheet (MSDS)
4. Provide one sample of COA (Certificate of Analysis)

QUESTION 4: Does your company have ISO 9001 certificate?

QUESTION 5: Please provide the lead time (days) your company can deliver the chemical to NSRP site when receive the order?
