SURVEY QUESTIONNAIRE FOR COMMUNITY HEALTH DEVELOPMENT PROJECT 2025 – 2027

Subject: Request for Quotation (RFQ) for Community Health Development Project 2025 – 2027

PURPOSE:

To shortlist bidders from the long-listed suppliers approved in the Bidding Strategy for the ‘Community Health Development Project (2025–2027)’ package, in compliance with NSRP’s Procurement Procedure, the Pre-Qualification Questionnaire has been developed as follows.

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is preparing to implement the Community Health Development Project (the Service) and is seeking a suitable supplier to provide the Service for a three-year period, from 2025 to 2027. The Service is tentatively scheduled to commence in June 202.

If your company is interested in this procurement package, please respond to all the questions in the questionnaire in Part 1 – Survey Questionnaire and send through email hau.ptt@nsrp.com.vn. The email must reach to the email address above no later than 10^th Apr 2025.The response submitted via email must be clearly marked with the following tittle:

“Community Health Development Project 2025 – 2027”

Part 2. Survey Questionnaire

QUESTION 1: NSRP have demand to seek the supplier to implement “Community Health Development Project from 2025 to 2027. Please refer to scope of work for the detail list of each category.

Could you confirm which category(ies) you intend to supply to NSRP?

• Conduct health screening for local people aged 30 and up (including: height and weight measures, hypertension, diabetes, abdominal ultrasound, Ear – Nose – Throat (ENT) endoscopy, chest X-ray, and health consultation.

•        Conduct health screening for local women aged from 18 to 60 (including: gynecology check, early screening test for cervical cancer, and health consultation).

• Conduct Health screening for pupils at primary & secondary schools (including eyes check, Ear – Nose – Throat (ENT) endoscopy, and health consultation).

• Conduct dengue fever prevention by communication activities and mosquito spray and repellent.
