Please submit the following qualification information and data in full in the same sequence as set out hereafter, which may be surveyed if it is required.

ORGANISATION AND CAPABILITY

2. Please state the company authorized and paid-up capital, equity distribution and names of shareholders.

3. Please state main activities of your company together with company profile literature.

4. Please provide your company organization chart.

5. Please provide evidence as to financial strength, including the following:

a) Banker’s reference

b) Audited Financial Statement of the Company for last 3 years

6. Have your company or associated your company been involved in any litigation?  If you have, please state the nature of litigations.

7. If your company participate in the bidding of this package, does your company need sub-contractor, or work as consortium or work independently.

EXPERIENCE & SUPPLY CAPABILITY OF YOUR COMPANY

1. Do you (more than two years) supply wooden pallet in Vietnam for more than 2 customers now?

If yes, please provide list of your customers with supplying time.

2. Could you provide maximum manufacturing capacity of wooden pallet per month by your company with requirement as Attachment 1?

3. Do you have full capacity to supply the required volume required (30,000 pcs/month) to NSRP with detail raw material source and production plan

4. Currently, can you provide wooden pallet designed as Attachment 1 including ISPM-15 treatment?

5. Where is your supply base located? Where are your actual pallet manufacturing locations?

6. How many days do you need to deliver product to NSRP site after receiving official order from NSRP? Will you deliver product to NSRP site?

7. Do you have Legal Certificate (of any kind, including contract, agreement, license, etc…) to supply raw materials including specifications of wood, sources of raw materials.
If yes, please clarify and send us scanned copies documents.

IMPLEMENTATION PLAN

1. Do you have organization for Quality Assurance in your company? If yes, please show name and main activities of the organization.
