SURVEY QUESTIONAIRE AND SCREENING CRITERIA CAVITATION CLEANING SERVICE FOR 061-E-303 & 061-E-503

Introduction.

NSRP is planning to establish a frame contract for yearly cavitation and water-jetting cleaning for heat exchangers of PPU Unit, with drawings as attached. The scope of work includes the cleaning and restoration of the heat exchangers to ensure reliable operation and maintain their optimal thermal performance.

Below is the survey questionnaire for selection of shortlisted bidders for the bidding process. If you have interest in supplying the service for NSRP, please kindly submit your Expression of Interest (EOI) via email address linh.ltt@nsrp.com.vn with title templated as follows “COMPANY NAME – EOI – MS260704 – Cavitation Cleaning Service of PPU Heat Exchangers” no later by Aug 24, 2026.

Part 1. Survey Questionnaire

QUESTION 1: Does your Company have ISO 9001 and HSE policy?

       If yes, please provide a notarized copy of these documents.

QUESTION 2: Does your company have a valid business license and company profile relevant to high-pressure water jetting and cavitation cleaning services?

1. Yes

If yes, please provide notarized copies of the business license, company profile, and relevant supporting documents.

QUESTION 3: Has your company successfully completed at least one (01) cavitation cleaning service within the last five (05) years?

If yes, please provide contract references, client acceptance certificates, or other supporting documents.

QUESTION 4: Can your company provide a commitment letter confirming completion of services for both heat exchangers (061-E-303 and 061-E-503) within three (03) days?
