SURVEY QUESTIONAIRE AND SCREENING CRITERIA – FOR PURCHASING THE: OFFLINE CONDITION MONITORING SERVICE FOR ROTATING EQUIPMENT

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procurement of Offline Condition Monitoring Service for Rotating Equipment to select suitable supplier for supply the goods.

1	Has your company provided similar Condition Monitoring services for rotating equipment within the last 3 years? ☐ Yes                                     ☐  No If ‘Yes’, please provide further details
2	Does your company have experience in working PRUFTECHNIK vibration device. ☐ Yes                                     ☐  No If ‘Yes’, please provide further details
3	Does your company have experience using OMNITREND Center and OMNITREND Center Utility to establish a condition monitoring program and perform diagnostics? ☐ Yes                                     ☐  No If ‘Yes’, please provide further detail.
4	What version of OMNITREND Center and OMNITREND Center Utility, license type your company, or your service engineer have been used. ☐ Yes                                     ☐  No If ‘Yes’, please provide further details
5	Do your company have at least 2 vibration specialists CAT III minimum who qualified by Mobius Institute or Vibration Institute. And with a minimum of 8 years’ experience in the Oil & Gas, Petrochemical, or Power sectors. Each engineer must have at least 5 years’ experience in vibration monitoring and diagnostics for plants or projects valued at over USD 1 billion, with more than 500 rotating equipment items (steam turbines, centrifugal compressors, multi-stage pumps, etc.)? ☐ Yes                                     ☐  No If ‘Yes’, please provide nominate of service engineer (certificate ID, CV).
