SURVEY QUESTIONAIRE AND SCREENING CRITERIA – Repair Service of Pressure Relief Valves and Manual Valves

Nghi Son Refinery & Petrochemical LLC (NSRP) is looking for long-term service providers of repair services for pressure relief valves (described as but not limited to: conventional, bellows type, pilot-operated PRV, and pressure-vacuum/breather valves) and manual valves (described as but not limited to: globe, gate, ball, plug, butterfly, knife gate, angle, check, diaphragm, soot blower poppet valve, automatic recirculation valve,..).

Please, kindly note that any submissions after the deadline may not be accepted. Documents shall be submitted in pdf/excel/word files, no link would be considered for the screening.

The submission shall be titled as per following template “NAME – EOI – MS260308 – Repair Service of Pressure Relief Valves and Manual Valves”.

Part 1. Survey questionnaires

Question 1: Does your Company have ISO 9001 Series certified and ISO 14001 certificate?

1.         Yes

2.         No

If yes, please provide a notarized copy of these documents.

Question 2: Does your Company have at least (05) years of experience in repairing service in time range from 2019 up to now for manual valves and pressure relief valves in industry?

1.     Yes

2.     No

If yes, please provide the notarized copy of Contract or client acceptance records.

Question 3: Does your Company have workshops for overhaul relief valves and manual valves with the minimum quantity as below “Attachment I – Valve workshop equipment requirement”?

1.     Yes

2.     No

If yes, please provide supporting documents such as workshop layout, along with a brief description of your workshop facilities, equipment, and capacity.

Question 4: Is your company able to provide laser welding and cladding services to NSRP?

1.    Yes

2.    No

If yes, please provide the technical specifications of the machines and equipment your company intends to utilize in providing services to NSRP.

Question 5: Does your company have manual and relief valve repair procedures, ITP/QCR report?

1.    Yes

2.    No

If yes, please provide the repair procedures with ITP/QCR record.

Question 6: Does your company workshop facilities have the capability to handle repairing 05 20-inch valves and 01 48-inch valve at the same time?

Question 7: In urgent cases, will you be able to repair a 30-inch valve within 18 hours? Yes/No.

If not, how long do you take to repair a 30-inch valve?
