SURVEY QUESTIONAIRE – FOR PROVISION OF TANK VERIFICATION SERVICE

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procurement of Tank Verification Service to select suitable Service Provider for service provision.

For the avoidance of doubt, this notice does not constitute any contractual offer, binding promise or guarantee of business by NSRP.

QUESTION 1: Does your company hold a valid Enterprise Registration Certificate (ERC), the relevant registered business line(s) and the required licenses (such as the appointment of verification services for vertical cylindrical tanks issued by Directorate for Standards, Metrology and Quality of Vietnam – STAMEQ) demonstrating eligibility to supply tank verification service?

If yes, please provide a notarized copy of these documents /enterprise registration certificate, business lines and required licenses with providing the following information

QUESTION 3: Can your company provide profile and organization structure to demonstrate the capability to supply similar service to customers?

If yes, please provide documents.

QUESTION 4: Does your company have similar contracts/ POs for supplying tank verification service to Oil & Gas industry / Terminal projects within the last 05 years?

1. Yes
2. No

If yes, please provide supporting documents: (1) a list of similar contracts/ POs and (2) At least one (01) Un-priced Contract/PO as supporting documents.
