SURVEY QUESTIONAIRE – FOR PURCHASE FENDERS AND NAVIGATION SPARES

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procurement of Fenders and Navigation Spares to select suitable Supplier for supply the good.

For the avoidance of doubt, this notice does not constitute any contractual offer, binding promise or guarantee of business by NSRP.

Part 1. Survey questionnaire

QUESTION 1: Does your company hold a valid Enterprise Registration Certificate (ERC) and the relevant registered business line(s) demonstrating eligibility to supply spare parts?

If yes, please provide a notarized copy of these documents /enterprise registration certificate and business lines with providing the following information

QUESTION 2: Is your company certified to ISO 9001?

1. Yes
2. No

If yes, please provide a valid certificate.

QUESTION 3: Can your company provide profile and organization structure to cover the supply of marine/ offshore spare part and after-sales service to customers?

1. Yes
2. No

If yes, please provide documents.

QUESTION 4: Has your company or any affiliated company failed to perform, breached, or been unable to complete any contract within the past 5 years?

1. Yes
2.  No.

If no, please provide (1) a signed commitment letter stating that your company has not failed to perform, breached, or been unable to complete any contract in the past 5 years and (2) attach the list of contracts with contact information of the customer for reference and verification. 

QUESTION 5: Does your company have similar contracts/ POs for supplying marine/offshore spare parts to Oil & Gas industry / Terminal projects within the last 05 years?

If yes, please provide supporting documents: (1) a list of similar contracts/ POs and (2) At least one (01) Un-priced Contract/PO as supporting documents

QUESTION 6: Can your company provide commitment to supply the goods with original/ copy Certificate of Origin (CO), Certificate of Quality (CQ) issued by Manufacturers or others and Warranty Period is at least 12 months warranty from date put in service or 18 months warranty from the date of handover the goods to NSRP, whichever comes first.
