SURVEY QUESTIONAIRE – FOR SELECTION SUPPLIER OF “ENGINEERING, PROCUREMENT, CONSTRUCTION, PRE-COMMISSIONING AND COMMISSIONING FOR UPGRADING CONTRACTOR GATE 8”

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procure of “Upgrading contractor Gate 8”

Subject: “Respond for Questionnaire Provision of Upgrading Contractor Gate 8 – by 15:00AM 20 March 2026 at the latest. For the avoidance of doubt, this notice does not constitute any contractual offer, binding promise or guarantee of business by NSRP.

1. General Scope of Work

The project shall consist of main facilities to satisfy its operation’s purpose as the scope of work is divided as below:

[1] Contractor Gate 8: To be upgraded based on existing infrastructure to enhance functionality and integration. Key requirements include:

• Expansion to 14m width with 4 vehicle lanes and ACS system.
• Vehicle access: 4 barrier gates with traffic control systems.
• Pedestrian access: 8 turnstile gates with shelters and access control.
• New pedestrian walkway from Road 513 with handrails.
• Systems included: drainage, fencing, power, lighting, CCTV, telecom, earthing & lightning.
• Demolition of existing guard shelters and related equipment

[2] Guard House: Area 25 m2 includes 01 security room and 1 toilet.

[3] Vehicle Inspection Area: a Vehicle inspection area of approximately 5,000m² which include:

• a vehicle parking area which is designed to accommodate 20 slots.
• a 25 m² inspection shelter with 40 m² eave for weather protection.

[4] Road & Fence include:

• A new road (through the existing car park) will be connected from road 513 to Contractor Gate 8 and the inspection area.
• 02 new emergency gates

[5] Piping systems include:

• Water supply system
• RFW piping system.

[6] Electrical system:

• Lighting and small power system
• HVAC Equipment
• UPS power supply for Telecom & Fire Alarm system in Guard House
• Earthing & lightning

[7] Telecom & Network: PAGA system, ACS & CCTV system, Telephone & LAN.

[8] Instrument System: Fire Alarm system is installed in Guard house and be integrated to FGS system

[9] Firefighting system

• Portable fire extinguishers are set up and arranged.
• Additional fire hydrants and fixed water monitors, hose cabinets are equipped & tie-in to existing RFW piping.

2. General / key requirements for Contractor capability (questionnaires)

[1] Have you ever performed the EPC service (or part of the EPC service, i.e. E or P or C) which involved the design, procurement, installation, pre-commissioning and commissioning for similar scopes (including Civil, Piping, Electrical, Instrument works) in Industry plant (Power plant, Refinery, Chemical, Industrial zone, etc.)? Y/N

If yes, please provide the details and evidence.

[2] Do you have experience in obtaining permits from Government Authority: List of executed contracts for similar work (include obtaining Detail planning 1/500 approval, Firefighting approval, Construction permit)? Y/N

If yes, please provide the details and evidence.

[3] Does your company (including Consortium member, but not Subcontractor) have Business License/Enterprise Registration Certificate which includes the work to be performed as set out in the Scope of Work (Engineering, procurement, construction, commissioning service)? Y/N

If yes, please provide the details and evidence.

[4] Are you interested in providing this EPC service on your own / or with other partners as a consortium? If yes, Bidder shall submit the Memorandum of Understanding (MOU) or agreement in case of a consortium or sub-contract.

If you are consortium, please provide the proposed split scopes (Parties involved, Responsibilities of Each Party, Scope of Collaboration), and qualification profile of your partners.

If you are subcontracting, please provide scopes and qualification profile of your subcontractors.
