SURVEY QUESTIONAIRE FOR SUPPLY OF RISK MANAGEMENT PMI TRAINING

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procurement of Expert development consultancy to select suitable supplier for supply the service.

If your company is interested in those procurement packages, please fill in and the complete questionnaire below then send email to anh.nt3@nsrp.com.vn “Response for Questionnaire “Risk Management PMI training” by 10 Mar 2026 at the latest. For the avoidance of doubt, this notice does not constitute any contractual offer, binding promise or guarantee of business by NSRP.

Please answer the questions below:

1. Do you have a business/enterprise registration certificate for training activities? If yes, please provide a soft copy. 

2. Do you have any official partnerships or authorizations from PMI to conduct training its risk management courses?

3. Have you conducted any training courses related to PMI Risk Management? If yes, please list notable courses and client names (if shareable).

4. How many years of experience do the instructors/trainers have in the field of Risk Management? Please kindly provide evidence (any certificate of service completion with other clients, etc…).

5. Are you willing to do this project between Jul and Sep 2026?
