Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is preparing for the procurement of supplying poly bag films for NSRP with detail information as Attachment 1- SOW

  QUESTIONAIRE

Please submit the following qualification information and data in full in the same sequence as set out hereafter, which may be surveyed if it is required. 

1. ORGANISATION AND CAPABILITY
2. Please provide full name of your company, correspondence and registered address of your company.
3. Please provide registration the copy of certificate of the company.
4. Please state the company authorized and paid up capital, equity distribution and names of shareholders.
5. Please state main activities of your company together with company profile literature.
6. Please provide your company organization chart.
7. Please provide evidence as to financial strength, including the following:
8. a)Banker’s reference
9. b) Audited Financial Statement of the Company for last 3 years
10. Have your company or associated your company been involved in any litigation?  If you have, please state the nature of litigations
11. EXPERIENCE&SUPPLY CAPABILITY OF YOUR COMPANY
12. Please show your existing customer’s processing condition about poly-bag film for high speed automatic FFS (Form, Fill and Seal) Bagging Machine (fill your answer into Attachment-3 file). It should be included your group companies (Parent company, subsidiaries of you and/or your parent company) in Asia.
13. Can you provide poly-bag film designed as Attachment-2?
14.       Can you modify the film properties to fit our bagging condition referring the result of test operation using the sample which you will supply?
15. Can you provide poly-bag film designed as Attachment-2 from July 2019 with 2,000 t/year volume? If it is difficult to supply all of volume how much volume can you supply?
16.       Please provide every location of your supply base (factory and/or warehouse) in Vietnam now.

2. Please provide evidence of certification for ISO 9001 and ISO 14001.
