Question 1: Does your company have Notarized Copy of the business/enterprise registration certificate for spare part supply?

Question 2: Is your company an OEM’s authorized distributor or manufacturer of any material in NSRP request list?

If yes, please provide copy of the documents and list out the related items.

Question 3: Shall your company provide CO (Certificate of origin), CQ (Certificate of quality), WT (Warranty certificate) for Goods?

If yes, please provide a confirmation letter (need evidence from OEM through common practice on OEM system or through email for providing of certificate such item), mention clearly for CO issue by manufacturer or Chamber of commercial, original or copy and why copy, how NSRP can confirm the certificate is real from OEM?
