SURVEY QUESTIONAIRE AND SCREENING CRITERIA FOR SERVICE BEARING REFURBISHMENT

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procurement of service refurbishment bearing to select suitable supplier.

For the avoidance of doubt, this notice does not constitute any contractual offer, binding promise or guarantee of business by NSRP.

QUESTION 1: Does your company have the licenses/ certificate /or permits for Service Bearing Refurbishment?

If yes, please provide a notarized copy of these documents.

QUESTION 2: Does your company have any Contract for service refurbishment bearing for compressor or steam turbine, heavy-duty pump in oil and gas industry?

1. Yes
2. No 

If yes, please provide a list of contracts (at least 3 contracts) for provision of service refurbishment bearing in oil and gas industry in the last 3 years.

QUESTION 3: Does your Company have ISO 9001 Series certified or/and ISO 14001 certificate?

1. Yes
2. No

If yes, please provide a copy of ISO certificate.

QUESTION 4: Please provide the lead time of your company for supply service refurbishment bearing?

1. Less than 10 weeks
2. From 10-14 weeks
3. More than 14 weeks
