SURVEY QUESTIONAIRE AND SCREENING CRITERIA FOR SUPPLY OF AMMONIA

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procurement of General Chemical to select suitable supplier for supply the good.

If your company is interested in those procurement packages, please fill in and the complete questionnaire below then send email to dung.nt7@nsrp.com.vn  “Response for Questionnaire “Supply of Ammonia” by 20 Dec 2024 (Hanoi time) at the latest. For the avoidance of doubt, this notice does not constitute any contractual offer, binding promise or guarantee of business by NSRP.

Part 1. Survey Questionnaire.

QUESTION 1: Does your company have the business licenses and/or permits authorized letter (in case of Bidder is not Manufacturer) in supplying ammonia?

If yes, please provide the notarized copy of these documents.

QUESTION 2: Does your company have any reference contract for provision of ammonia supplying to any company?

1. Yes
2. No 

If yes, please provide list of contracts for provision of ammonia in last 3 years.

QUESTION 3: Does your Company have ISO 9001 Series certified and ISO 14001 certificate?

1. Yes
2. No

If yes, please provide copy of ISO certificate.

QUESTION 4: Please confirm your company can supply ammonia by which options. If select option 2, kindly provide engineering design document for dilution system in including: Piping & Instrumentation Diagram, equipment data sheet.

☐ Option 1: Supply ammonia 25% solution to NSRP ammonia tanks by lorry

☐ Option 2: On-site production of NH3 25% solution from anhydrous NH3 99% including:

• Deliver ammonia 99% to NSRP site.
• Provide dilution system to dilute ammonia from 99% to 25% at NSRP site before supply to NSRP ammonia tanks.
• Supplier is responsible for licenses of ammonia dilution activities at NSRP site in accordance with authorities’ regulations.
• Provide full service including operation & maintenance of dilution system.
• The required utilities such as electricity, water, air… will be supplied by NSRP and paid by supplier.
