SURVEY QUESTIONAIRE AND SCREENING CRITERIA

Nghi Son Refinery & Petrochemical Limited Liability Company (LLC) is planning to select suitable supplier for  supplying  TANK MATERIALS/ SELF ACTIVE VALVE OF SOOT BLOWER

If your company is interested in those procurement packages, please fill in and complete questionnaires below then send email to ha.tt1@nsrp.com.vn with title: “Company name_EOI_Tank materials/self-active valve of soot blower” no later than 19 May 2026. For avoidance of doubt, this notice does not constitute any contractual offer, biding promise or guarantee of business by NSRP.

Part 1. Survey Questionnaire.

QUESTION 1: Does your company have a business license which has a business sector in relation to material trading in the industrial?

If yes, please provide a notarized copy of these documents.

QUESTION 2: Does your company have any reference contracts for providing Tank spare part?

1. Yes
2. No

If yes, please provide at least 1 contract.

QUESTION 3: Does your company have authorization letters or official supporting documents for items such as swing joints and swivel joints from experienced manufacturers?

1. Yes
2. No

If yes, please provide a copy of the letter.

QUESTION 4: Does your Company have ISO 9001 certified?

1. Yes
2. No

If yes, please provide a copy of the ISO certificate.

Part 2. Screening criteria

Potential candidate is “Passed “and selected to form the long list if their answer is:

Answered “Yes” – for questions and No 1; 2; 3; 4 with sufficient evidence.
