Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for Purchasing Radiation Safety Services (the Services) as Attachment 1.

For the avoidance of doubt, this notice does not constitute any contractual offer, binding promise or guarantee of business by NSRP.

Part 1. Survey questionnaires

Please answer the questions below:

QUESTION 1: Does your company possess a valid Business Registration License authorizing the provision of the proposed services?

(As required under Atomic Energy Law No. 94/2025/QH15 and its implementing regulations.)

QUESTION 2: Is your company certified by the competent Vietnamese authority to perform radiation survey (radiation monitoring) services in accordance with applicable Vietnamese regulations, including Atomic Energy Law No. 94/2025/QH15 and Decree No. 142/2020/ND-CP?

If yes, please submit copies of the following documents:

• Radiation Survey / Radiation Monitoring Service License (Giấy phép kiểm xạ);
• Relevant supporting certificates;
• Documentation demonstrating compliance with personnel qualification requirements (Radiation Safety Officer, trained radiation workers);
• Equipment calibration certificates for radiation measuring instruments;
• Documentation proving compliance with radiation safety and technical requirements

QUESTION 3: Does your company hold a valid Radiation Operation License issued by the Ministry of Science and Technology (MOST) as required in Atomic Energy Law 94/2025/QH15 and Decree No. 142/2020/ND-CP?
