SURVEY QUESTIONAIRE FOR CONTRACT FOR VERIFICATION SERVICE FOR GROUP 2 INSTRUMENTS AND ANALYZERS

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procurement of – verification service for Group-2 instruments- verification service for Group-2 instruments and analyzers.

QUESTION 1: Does your company hold an Enterprise Registration Certificate (ERC) & Relevant Business Line Code demonstrating eligibility to provision of verification & relative accuracy testing services for analyzers and instruments of continuous emission monitoring system, wastewater monitoring system & verification service for Group-2 measuring instruments of tank gauging systems and metering systems?

2. No

If yes, please provide the notarized copy of your business/enterprise registration certificate and business code.

Note:  In accordance with the Law on Enterprises 2020 and Decree No. 01/2021/ND‑CP on enterprise registration, business lines are no longer displayed directly on the ERC. Therefore, NSRP will verify the Bidder’s registered business lines via the National Enterprise Registration Database.

QUESTION 2: Does your company have experience in providing verification & relative accuracy testing services for analyzers and instruments of continuous emission monitoring system, wastewater monitoring system & verification service for Group-2 measuring instruments of tank gauging systems and metering systems?

If yes, please provide the notarized copy of at least 01 full copies of similar executed contracts

QUESTION 3: Does your company have any technician/ personnel that under contracts with terms of 12 months or more certified to work on the inspection/calibration/testing services?

1. Yes

If yes, please provide us the evidence: CV and certificate for performing the required service of at least 03 technicians

QUESTION 4: Does your Company’s (or your Consortium’s) establish and maintain the management system in conformity with the National regulation TCVN ISO/IEC 17025 for inspection/calibration/testing of measuring instruments and measurement standards?
