SURVEY QUESTIONAIRE FOR SELECTION THE LONG LIST BIDDER OF “NSRP TA PACKAGE No.06” Package

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for the procurement of providing Turn Around Package No.06 Service. The brief scope of lump sum service is as follows:

• The service includes the provision of engineering, installation, maintenance, testing, modification, and technical support services for NSRP DCS, SIS, FGS and associated systems, including DCS/SIS/FGS functional testing and preventive maintenance, SIF proof testing, fiber optic and LAN testing, FTE cable replacement, battery replacement, firmware and Cisco IOS upgrades, SIS power and alarm modifications, etc… and related technical support.
• The Contractor shall provide qualified manpower, tools, equipment, testing, documentation, and as-built records necessary to complete the works in accordance with NSRP procedures, applicable standards, and OEM requirements.
• The service shall also include Work Pack preparation, PTW preparation (SWMST, JSA, etc…), work execution, and completion of pre- and post-TA documentation and related activities.

SURVEY QUESTIONNAIRES

• Does your company hold valid business registration, and all required legal licenses, permits, and eligibility to perform above mentioned scope of service system works in Vietnam?

Evidence to attach: Business Registration (PDF), relevant licenses/permits, company profile.

• Does your company hold valid ISO 9001 and ISO 14001 certifications?

Evidence to attach: Copies of valid ISO 9001 and ISO 14001 certificates, including the certificate number, scope of certification, issuing certification body, and validity period.

• Has your company provided maintenance services for control systems in refinery or petrochemical plants?

Evidence to attach: Relevant project references, contracts/POs, completion certificates, or client reference letters.

• Has your company experienced fiber optic (FO) testing, power supply unit (PSU) testing and control system maintenance activities during turnaround or shutdown projects at industrial plants within the last five years?

Evidence to attach: Relevant Turnaround/Shutdown project references and supporting documents (e.g., contracts/POs, completion certificates, or client reference letters) to demonstrate the experience.

• Has your company previously had, or currently has the capability to obtain, any formal authorization, certification, partnership, or service/support agreement with Honeywell to provide control system services for NSRP?

Evidence to attach: Copies of valid Honeywell authorization, certification, partnership certificates, service/support agreements, or other official documents demonstrating the company’s formal relationship with Honeywell.

• Does your company have experience complying with NSRP safety requirements and procedures during turnaround periods, including Permit to Work, Lockout/Tagout (LOTO), electrical safety, working at height, confined space entry, and other applicable HSE requirements?

Evidence to attach: Provide evidence demonstrating your company’s experience in preparing PTW documents at NSRP, and/or HSE training or competency certificates and refinery project references confirming personnel compliance with PTW, LOTO, electrical safety, working at height, confined space entry, and other applicable HSE requirements.

• Is your company familiar with NSRP TA Work pack preparation, field equipment locations and capable of effectively performing maintenance activities?

Evidence to attach: Relevant NSRP project references, personnel records, or work completion certificates demonstrating prior site experience and familiarity with equipment locations.
