SURVEY QUESTIONNAIRE Contract for Provision of Quantity and Quality Inspection Service

Nghi Son Refinery & Petrochemical Limited Liability Company (NSRP) is planning for procurement of “Contract for Provision of Quantity and Quality Inspection Service” hereinafter referred to as the Service to select suitable supplier for supplying the Service.

If your company is interested in those procurement packages, please fill in and the complete questionnaire below then send email to dung.nt7@nsrp.com.vn, state clearly “Response for Contract for Provision of Quantity and Quality Inspection Service” by 8 Sep 2026 at the latest. For the avoidance of doubt, this notice does not constitute any contractual offer, binding promise or guarantee of business by NSRP.

Part 1. Survey Questionnaire.

QUESTION 1 (Applicable to Scope No. 1,2): Does your company hold a valid Enterprise Registration Certificate (ERC), with the relevant registered business lines and the required licenses issued by authorized organizations, accreditations and approvals required by applicable laws and regulations to perform the inspection services? (ISO 17020, ISO 17025 or Decree 107/2015/NĐ-CP certificates, etc. …) demonstrating its eligibility to provision of required Scope of Services above table in compliance with applicable laws and regulations?

• In accordance with the Law on Enterprises 2020 and Decree No. 01/2021/ND‑CP on enterprise registration, business lines are no longer displayed directly on the ERC. Therefore, NSRP will verify the Bidder’s registered business lines via the National Enterprise Registration Database.
• The candidate may submit responses for any individual scope or combination of scopes based on Candidate’s qualifications and capabilities. Participation in all three scopes is not mandatory.

QUESTION 2 (Applicable to Scope No. 1,2): Does your company maintain quality management system in accordance with ISO 9001?

QUESTION 3 (Applicable to Scope No. 1,2): Can your company provide profile and organization structure to demonstrate the capability to supply similar service to customers?

QUESTION 4 (Applicable to Scope No. 1,2): Does your company have at least one (01) similar contracts/POs within the last five (05) years related to provision of similar services to Oil & Gas, Refinery, Petrochemical, Chemical or Energy industries?
